As an NDIS participant, your consultation fee will be invoiced to your plan manager or claimed directly from the NDIA. Please complete all fields below and sign the declaration before submitting.

Once we receive your booking request we will contact you to confirm your appointment and provide a quote. Please note that depending on your situation, your appointment may be covered by NDIS, a Home Care Package, or billed privately.
